W. (American; 1-yr exp; age 60; Married; $50.00 Insurance) Sept. 4, 1901. E. W. Simpson, Raleigh No. 1 Mine, was fixing to tamp hole when slate gave way and crushed him. He had tried to pull the slate down the evening before and thought it was safe. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Warren, Richard H. (English; 14-yrs exp; age 38; Married; $50.00 Insurance) Sept. 27, 1901. Richard H. Warren, Raleigh No. 1 Mine, had just fin- ished loading car of coal and was preparing to either timber or take down the slate, when he stepped to the face to remove his shovel and in so doing had to pass under the slate when it fell on him. He knew the slate was not safe. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Hooten, Chas. W. (American; age 14; Trapper; Single) Dec. 4, 1901, died Dec. 15, 1901. Chas. W. Hooten, Raleigh No. 1 Mine, had taken a bottle of carbon oil in mine in his pocket. It broke in his pocket and he took his lamp and set fire to his clothes, and then put them out. He did this several times and the last time he did it he could not put his clothes out and was burned so badly that he died from the effects of it. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Nestor, Everitt S. (American; age 17; Mach. runn'r; Single) April 25, 1902. Everitt S. Nestor, Raleigh No. 4 Mine, was loading car with coal. Kettle-bottom of sandrock fell from roof upon him, killing him instantly. When the roof was examined the day before it was apparently perfectly safe. The roof is all sandstone and nothing like this had ever been seen before. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Mitchem, Zeb (American; 2-yrs exp; age 25; Single) May 7, 1902. Zeb Mitchem, Raleigh No. 2 Mine, was engaged in boring hole in No. 2 Mine when piece of slate fell.
