     July 6, 1904. Green Pratt, Austen No. 2 Mine, was driving a cross-cut 
     between rooms. He evidently considered roof in safe condition on account 
     of it being narrow and did not use proper care in timbering. Roof in this 
     place is very brittle and rotten.

     coming out of mine on motor. He came in contact with trolley wires and 
     was thrown off motor and crushed between motor and rib.
