     Nov. 24, 1903. Toney Albert, Henry Shaft Mine, entered a place that
     he was instructed not to go into and was killed by an explosion of gas.

Joe, Frank

     Dec. 28. 1903. Frank Joe, Henry Shaft Mine, had his head caught
     between door and jam, of door of which he had charge. He had opened
     the door and then let it swing shut.

Krupha, Alex
Henderson, C. E.

     March 15, 1904. Alex Krupha and C. E. Henderson, Henry Shaft Mine
     No. 33, were killed by an explosion of gas. They were laying a
     switch in 1st left cross-cut and came in contact with a body of
     explosive gas. The place had been examined by the Fire Boss in the
     morning and was found free of noxious gases.
