Witherill, aged 53 years, a resident of Ossian, County of LIvingston, State of New York, who being duly sworn according to law, declares that he is the identical Isaac H. Witherill, who entered service during the War of the Rebellion under the name of Isaac H. Witherill on or about the 28 day of August, 1862, as Private in company B of the 136th regiment of N.Y. commanded by Captain _______ Pratt, and was HONORABLY DISCHARGED at near Washington, D.C., on or about the 13th day of June, 1865, by reason of close of the war; that his personal description is as follows; Age, 53 years; height, 5 feet 11 inches; complexion, Dark; hair, Dark; eyes, Grey. That he is now suffering from Partial paralyses of the left side of head & from heart trouble. and that the said disability is of a permanent character, and is not the result of vicious habits, and that it incapacitates him from the performance of manual labor in such a degree as to render him unable to earn a support, and that this declaration is made for the purpose of being placed upon the pension roll, under the provisions of the Act of June 27, 1890. That he has never been employed in the military or naval service otherwise than as stated above. That since the 13 day of June, A.D. 1865, he has not been employed in the military or naval service of the United States. He hereby appoints, with full power of substitution and revocation, GEORGE E. LEMON OF WASHINGTON, D.C., his true and lawful Attorney, to prosecute his claim. That he has never received but has applied for a pension. Claim #386843. Witherill the claimant sign his name to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with him, that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim. Isaac H. Witherell, a Private of Company B, 136th Regiment New York Volunteers, was enrolled on the 28 day of August, 1862, at Ossian for 3 years, and is reported: On Roll of the Co. for July and August 1863 present. Musterout Roll of the Co. dated June 13th, 1865, near Washington D.C. reports him mustered out with the Co. on that date. Not borne on Regimental return for July 1863 as absent Sick or other- wise. Regt was in action at Gettysburg Pa. July 2nd & 3 1863. The records of this office furnish no evidence of alleged wound. Regimental Hospital Records or Company Morning Reports are not on file. W.W. Witherell who was a Private Co. B 136th Regiment New York Volunteer Infantry is entitled to a pension at the rate of Twelve dollars per month to commence on the Twelfth day of July one thousand eight hundred and ninety. This pension being for: Slight deafness of both ears and disease of heart.
