On this 4 day of March, A.D. one thousand nine hundred and 7 personally appeard before me, a Notary Public within and for the county and state aforesaid, Charles Witherell, who, being duly sworn according to law declares that he is 77 years of age, and a resident of Ossian county of Livingston, State of New York; and that he is the identical person who WAS ENROLLED at Elmira under the name of Charles Witherell, on the 10 day of August, 1864, as a Private, in Co. G. 58 Regt. N.Y.S.N.G. in the service of the United States, in the Civil war, and was HONORABLY DISHCARGED at Elmira, on the 2nd day of December, 1864; That he also served [line left blank] That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 11 inches; complexion, light; color of eyes, blue; color of hair, Sandy; that his occupation was a Farmer; that he was born April 24, 1829, at Ossian Liv Co N.Y.
